RN Disease Management

The RN Disease Manager is a licensed Registered Nurse who has graduated from an accredited nursing program and holds a current, unrestricted license in any U.S. state or territory. Candidates must possess two years of recent, broad-based RN experience and certifications such as Certified Asthma Educator (AE-C) and/or Certified Diabetes Educator (CDE). This position is subject to military credentialing and privileging requirements. The RN works as part of a multidisciplinary team to coordinate and deliver disease management services for patients with chronic conditions, including asthma, diabetes, and chronic pain. Responsibilities include conducting health assessments, delivering patient education, managing telemedicine consults, and ensuring compliance with military treatment facility (MTF) standards. The role involves supervising care teams, administering clinical interventions, facilitating specialty referrals, and contributing to evidence-based research to support continuous improvement in patient care and outcomes.

QUALIFICATIONS

• Certifications: Certified Asthma Educator (AE-C) awarded by the National Asthma Educators Certification Board. Certified Diabetes Educator (CDE) awarded by the National Certification Board for Diabetes Education. Graduation from an accredited nursing educational program. Possess and maintain a current unrestricted license to practice as an RN in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.

• Experience: Possess two (2) years of full-time broad-based registered nurse experience within the preceding five (5) years.

• Licensure/Registration: Current, full, active, and unrestricted license to practice as a Registered Nurse

Job Duties and Responsibilities:
The duties include but are not limited to the following:
• Participate in multidisciplinary team with other health care providers providing consultative services pursuant to optimal health care delivery to patients suffering from pain.
• Participate in tele-medicine and tele-consults.
• Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.
• Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
• Perform physical exam and health histories.
• Provide health promotions, counseling, and education.
• Administer medications, wound care, and numerous other personalized interventions.
• Direct and supervise care provide by other healthcare professionals.
• Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
• Recognize adverse signs and symptoms and quickly react in emergency situations.
• Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
• Make referral appointments and arrange specialty care as appropriate.
• Perform nursing services identified in the TO.
• Conduct research in support of improved practice and patient outcomes.

Monday through Friday between the hours of 0730 and 1600.
The HCW shall be scheduled across the psychiatric clinic operating hours on a flexible basis. Generally, shifts are scheduled for an eight and a half (8.5) hour period (including an uncompensated thirty (30) minute meal break), or the HCW may work four (4) ten and a half (10.5) hour periods (including an uncompensated one (1) hour meal break).
