  • Degree/Education: Doctor of Pharmacy (Pharm. D.)/ Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE)
  • Internship/Residency: Completion of a clinical pharmacy residency OR fellowship accredited by the American Society of Health System Pharmacists OR American College of College of Clinical Pharmacy OR documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy OR Board certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties.
  • Certifications: BLS
  • Experience: HCW must have practiced (Clinical Capacity) for a minimum of 1 year within the last 2 years. For the Clinical Pharmacist at the Anticoagulation Clinic, HCW must demonstrate competency in Anticoagulation Therapy during phone interview, if requested
  • Licensure/Registration: Current, full, active, and unrestricted license to practice as a pharmacist.

The duties include but are not limited to the following:

  • Perform a full range of pharmacist procedures.
  • Assess patient’s response to drug therapy and planning drug therapy based on physician-established diagnoses.
  • Order and evaluate laboratory test necessary to evaluate drug therapy effect and outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with medical staff.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic).

Duty Hours: 7:30 AM – 4:30PM Monday – Friday, Shifts could include but are not limited to a 9-hour shift which includes a 1-hour non-paid lunch each day, or 12 hours on a panama schedule which includes 2 days on, 2 days off, 3 days on, 2 days off, or 2 days on 3 days off, not to exceed 80 hours per two weeks.
